Have your live scan request form ready. The booking questions below ask for your ORI number and agency name — both are on your request form. After you book, your confirmation email will include a link to forward a photo or PDF of the form to [email protected] so we can pre-print your info.
Your request form
Bring the form from your agency (BCIA 8016 or equivalent). We need the ORI number to route correctly.
Current photo ID
Unexpired driver's license, state ID, US passport, or military ID. Name must match your request form.
Payment
$30 rolling fee plus the government processing fees listed on your form (typically $40–$90).
